7.) Certificate of Graduation/Diploma and Transcript
8.) 1 Photo (passport size)
9.) $200 Admission Fee. (Make your check payable to Life University)
10.) Copy of ID Card (i.e; State ID/CA ID, if any)
11.) Essay (Regarding your personal interests, experience, strengths and goals. Include the reason why you choose Life University).
12.) Copy of OPT (if any)
